Pravastatin, Rosuvasatin,Simvastatin are formulary
For patients on concomitant gemfibrozil, the formulary substitution
will be automatically reduced to simvasatin 10 mg daily
For patients on concomitant amiodarone or verapamil, the formulary substitution
will be automatically reduced to simvasatin 20 mg daily
For patients intolerant to simvastatin may use pravastatin
